At Alert Level 2 we ask that you let us know if:

  1.  You have any Covid-19 symptoms 

  2.  You have been in contact with anyone who has Covid-19

  3.  You have travelled from overseas in the last 14 days.

If any of these apply we will need to reschedule your treatment.

  • ​We ask that you come on time and alone to your appointment if possible. If you need a support person, please let us know in advance.

  • ​If you are a new patient, please complete the on-line form below before your appointment.
